At Convergence Vitality, we operate as a cash-based practice to provide care centered around the patient — not the limitations often imposed by insurance companies. This model allows us to spend more one-on-one time with each patient, create highly individualized treatment plans, and focus on the quality of care rather than visit restrictions or productivity demands.

Many complex conditions such as Ehlers-Danlos Syndrome (EDS), dysautonomia, POTS, concussions, and chronic orthopedic conditions require comprehensive care that often extends beyond what insurance companies consider “standard.” By remaining out-of-network, we are able to tailor treatment sessions to your unique needs, integrate multiple treatment approaches, and progress care based on your goals rather than insurance approvals.

To help patients utilize potential out-of-network benefits, we can provide a superbill (when requested) that may be submitted directly to your insurance company for possible reimbursement. While reimbursement is not guaranteed and varies by plan, many patients are able to receive partial reimbursement depending on their out-of-network coverage.

  • A Physical Therapy Initial Evaluation is a comprehensive assessment performed by a licensed physical therapist to better understand your condition, symptoms, medical history, movement limitations, strength, mobility, and functional goals. During this visit, your therapist will discuss your concerns, perform a detailed examination, and develop an individualized treatment plan tailored to your specific needs and goals. An initial evaluation is required for all new patients and for returning patients who have not been seen by their provider within the last 6 months. Patients who have been seen within the last 6 months may schedule a follow-up appointment without a new evaluation.

  • a follow-up session is designed for established patients seeking a single follow-up treatment session, reassessment, progression of exercises, symptom management, or guidance related to a previously treated condition. This visit may include hands-on treatment, therapeutic exercise progression, mobility work, education, and updates to your home program based on your current needs and goals.

  • A Telehealth Physical Therapy session allows patients to receive virtual physical therapy services from the comfort of their home through a secure online platform. Telehealth visits may include guided therapeutic exercises, movement assessment, symptom management, education, home exercise progression, activity modifications, and discussion of recovery goals and progress.

  • A Dry Needling Session is designed for established patients seeking targeted dry needling treatment for muscle tightness, trigger points, pain, or mobility restrictions. This appointment is focused specifically on dry needling treatment and may address 1–2 treatment areas during the session.

    To schedule this appointment type, patients must have completed an initial evaluation within the last 6 months.

  • A Follow-Up Physical Therapy Package is designed for established patients who are actively participating in a plan of care and require ongoing treatment to achieve their rehabilitation, performance, or wellness goals. Follow-up visits may include reassessment of symptoms and progress, therapeutic exercise progression, manual therapy, neuromuscular re-education, mobility training, patient education, and updates to your home exercise program. Purchasing a package allows patients to commit to their treatment plan while receiving a discounted rate compared to individual sessions. Patients must have completed an initial evaluation or have been seen by their provider within the last 6 months to be eligible for follow-up packages.
